FAITH FORMATION REGISTRATION (GRADES 1-10)

  Thank you for registering your children at St. Florence for Faith Formation. We are grateful to be a part their education in our Catholic faith and about the life of Christ.
  
  Please note, St. Florence will only offer a Faith Formation program for this upcoming calendar school year (2022/2023). We will be transitioning to a Shrine for Healing later this fall. Once completed, Canon Law, will no longer allow us to offer a Faith Formation program. Even though we will be officially a Shrine in the fall, we will complete this Faith Formation instruction year.

- Grade 2 will meet on Tuesday evenings.
  
  Grade 10 will meet once a month on Sundays at 6PM
  
  All other grades will be a hybrid homeschool with periodic in-person meetings.
- Registration Cost
  
  One child - $185
  
  Two children - $285.00
  
  Three or more children - $335.00
  
  Payment can be made via check, made to "St. Florence Parish" and include "Faith Formation" in the memo. You can drop off/mail your check to the office. (For drop off outside of business hours, place in slot in door.)

  I, as the parent or guardian of the child(ren) I have registered, authorizes the staff and personnel of St. Florence Parish to treat my son or daughter in case of emergency. Further, I release the staff, personnel, and volunteers of St. Florence Parish from any act or omission conducted in the course of rendering such care to my son or daughter. In the event of an emergency, your child will be taken to the nearest hospital, unless otherwise notified. In case of a medical emergency, I give permission for my child to be treated by a physician.
